Birth Certificates

Birth certificates are not public record.  There are a limited number of people who may apply to receive a certified copy of a birth certificate.  Access to these records is limited to:

  1. The person listed on the certificate;
  2. The parents shown on the birth record;
  3. An authorized legal guardian or agent;
  4. Grandparents of the person named on the certificate; 
  5. An adult child of the person named on the certificate;
  6. An adult sibling of the person named on the certificate;
  7. The spouse of the person named on the certificate.

** You must have a valid picture identification card to obtain a certified copy of a birth certificate.**

 

Death Certificates

There are a limited number of people who may apply to receive a certified copy of a death certificate.  Access to these records is limited to:

  1. The person listed on the certificate;
  2. The parents shown on the birth record;
  3. An authorized legal guardian or agent;
  4. Grandparents of the person named on the certificate;
  5. An adult child of the person named on the certificate;
  6. An adult sibling of the person named on the certificate;
  7. The spouse of the person named on the certificate.

O.C.G.A. 31-10-15 et. seq. and DHS Rules 290-1-03 prohibit the release of confidential medical information to anyone except next of kin and other authorized persons.

 

Fees for Birth and Death Certificates

There is a $25.00 fee for the first copy and $5.00 for each additional copy per visit.

Fees are payable in cash or money order only.  NO PERSONAL CHECKS WILL BE ACCEPTED.  An application listing general information (which can be found at the bottom of this page) must be filled out by the requester on both birth and death and certificate requests.

 

Request Certified Copies by Mail

You may request a birth or death certificate via mail by completing the form at the bottom of this page.  Mail the completed form along with the following:

  1. A self-addressed, stamped envelope;
  2. A money order for the total amount of certificates you are requesting;
  3. A copy of your identification.  If you are a legal guardian or personal representative, you must enclose a copy of the order showing your appointment as such.
