Request for Hearing
A request for hearing form may be completed and submitted for filing with the Clerk of Court no less than 30 days after service is rendered.
Before submitting a request for hearing the following must be completed:
- Completion and filing of all documents related to your case.
- Proof of service of the complaint/petition to opposing party (or in the alternative the opposing party has completed an acknowledgment of service) has been filed with the Clerk of Court.
A request for hearing form may be completed and submitted for filing with the Clerk of Court no less than 30 days after service is rendered.

- The request for hearing along with the case file will be sent to the judge’s office for review. If the file is complete the judge’s office will set a hearing and notice of the hearing will be sent to the parties by mail at the addresses on file with the Clerk of Court. If the file is incomplete a letter will be sent by the judge’s office to the parties indicating missing information and further instruction.
